KEYWORDS Husband's Attendant, Anxiety Level, and Childbirth INTRODUCTION Pregnancy brings various physical and psychological changes, so that a calm physical and psychological condition is needed so that the process of pregnancy and childbirth can run well. For primigravida is a first experience, a pregnant woman who is waiting for the process of giving birth or giving birth to a baby into the world. Childbirth is a process in which a woman gives birth to a baby that begins with regular uterine contractions and peaks at the time of expulsion of the baby until the expulsion of the placenta and its membranes where this labor process will last for 12 to 14 hours (Kurniarum, 2016). In the first stage of Childbirth, many problems occur to mothers who are about to give birth, such as difficulty sleeping, fear, loneliness, stress, anger, tiredness, disappointment, feelings of hopelessness, especially anxiety about childbirth (Murray and Gayle, 2013). Maternal mothers have excessive emotions that cause high anxiety, a situation where mothers always think about bad things that might happen. Based Word Healt Organization 2015, There was pregnant women who experience anxiety 99% occurred in developing countries. Depkes RI (2016) states that in 2016 there were 107 million (28,7%) people in Indonesia pregnant women experienced anxiety in the face of childbirth. Research conducted by primigravida 22,5% experiencing mild anxiety, 30% moderate anxiety, 27,5% seriously worried, and 20% experiencing severe anxiety (Sarifah, 2017). Research conducted in Surabaya, Java Timur2016 as much as (41,7%) mother experiencing anxiety before childbirth. This showed that primigravidas who experience childbirth had a greater risk of experiencing anxiety compared to multigravida who was had previous childbirth experience (Tamala, 2020). Reducing anxiety during childbirth was the presence of a companion, such as a husband. The presence of a second person could provide comfort during childbirth. The presence of a birth attendant could have a positive effect on childbirth, which could reduce morbidity, reduce pain, shorten labor, and reduce the rate of cesarean childbirth (Marmi, 2016). METHODS The research design used a quantitative research design with the type of correlational analytic research. Research with a research approach cross sectional. The population were of all primigravida who examined and had an estimated childbirthin July to August 2022, at Health Primary Care Centre Kunir Residence as many as 40 people. Data collection used a questionnaire. Determine whether there was a relationship or not used the chi square test with a significance level of ≤ 0.05. RESULTS 1. Meanwhile, 7 respondents (19.44%) without being accompanied by their husbands experienced severe anxiety levels. DISCUSSION 1. Husband's Companion in Maternity Table 5.4 From 36 respondents, it can be seen that most of the mothers gave birth with their husband's companions as many as 29 people (80.55%), and primigravida without husband's assistance as many as 7 people (19.44%). A husband's companion was a husband who accompanies or accompanies his wife in the birth process (Bobak, dkk, at Marmi, 2016). Mothers who were about to give birth are quite a difficult period so they needed the support and assistance of their husbands in the process of giving birth until they give birth safely and comfortably (Siti, 2016). The presence of a birth attendant could have a positive effect on childbirth, could reduce morbidity, reduce pain, shorten childbirth, and reduce the rate of cesarean childbirth. The presence of a birth attendant could provide a sense of comfort, enthusiasm, emotional support, and could encourage the mother (Jannah, 2017). Maryunani 2015, The role of the birth attendant, namely the birth attendant, could help the mother divert attention from the pain that had started to appear, The birth attendant could remind the mother to drink every few hours and urinated every two hours, when pain or contractions arise, the birth attendant could invite the mother to talk while giving praise if the mother passes every contraction that occurs, the birth attendant could help the mother change body position when it starts to look stressed or tired, and the birth attendant could provide a gentle massage on the back of the mother's leg or shoulder. Many respondents were accompanied by their husbands during the childbirth process. It showed that the presence of good husband assistance could affect the process of giving birth to his wife. 2. Primigravida Anxiety Levels in Facing the Childbirth Process The process mostly experienced mild anxiety levels, 16 people (44.44%), moderate anxiety, 13 people (36.11%), and 7 people (19.44%) experienced heavy anxiety. Solehati & Cecep, 2015 states that anxiety was a mood disorder characterized by feelings of deep and ongoing fear, no disturbance in assessing reality, personality was still intact, and behavior was disturbed but was still within normal limits. Hawari, 2013 Anxiety was a mood disorder characterized by feelings of deep and ongoing of fear, not experienced disturbances in assessing reality, personality was still intact; behavior could be disturbed but was still within normal limits. Anxiety experienced by mothers during childbirth, mothers felt excessive pain. Fear would hinder the birth process because when the human body gets a fear signal, the body would activate the alert and defense center. As a result, the uterus would get a little blood flow so that it blocked the birth process and caused pain and caused the childbirth time to be longer. Mother would become more tired, lose strength, the opening would take longer. Feelings of fear during the labor process could affect the flow and smooth opening, so that it could interfere with the childbirth process (Mairida, 2018). Maternal anxiety levels were at the level of mild, moderate, and severe anxiety. The emergence of anxiety with mothers experiencing pregnancy for the first time and early in the face of childbirth, there was lack of knowledge and information about the birth process. 3. Analysis The Relationship of Husband's Attendant with Primigravida’s Anxiety in the First Childbirth Based Table 5.6 The results of the cross tabulation of husband's Attendant with Primigravida’s Anxiety in the First Childbirth process showed that of 29 respondents (80.55%) accompanied by their husbands with mild anxiety levels 16 respondents (44.44%), and moderate anxiety 13 respondents (36.11%) ). Meanwhile, 7 respondents (19.44%) without being accompanied by their husbands experienced severe anxiety levels. The Relationship of Husband's Attendant with Primigravida’s Anxiety in the First Childbirth significance value (p) that was 0, 00 ≤ α = 5% (p = 0,05). The calculated P value with the help of SPSS 16 analysis software with chi square analysis showed the results of Asymp Significant = 0.00 so that it had a research significance value with a degree of confidence 5%, P = 0.05 greater than the research P-count, it could be interpreted that H0 is rejected or H1 is accepted. This statement was equivalent to the meaning that there were The Relationship of Husband's Attendant with Primigravida’s Anxiety in the First Childbirth. The research calculated P value = .000 in the attachment of the chi square test there was no negative sign (-), so it can be interpreted that the relationship between variables was positive. This meant that the way to reduce primigravida anxiety in the face of childbirth was by assisting husbands who could provide love, feelings of security and comfort, and share the burden during childbirth. Based on research conducted by Nurpratiwi and Anggaresi 2018 at General Hospital YARSI Pontianak that was, it was known that between the husband's mentoring variable and the mother's anxiety level variable facing childbirth, the significance value was 0.038 (α<0.05), which meant that there was a relationship between husband's accompany and maternal anxiety facing childbirth. The presence of a companion during the work process could have a positive influence on the mother, with the presence of a husband companion, the mother could share the pain and the husband could calm pain to the wife and provided motivation to be stronger in the delivery process (Nurpratiwi and Anggaresi, 2018). Mothers who were accompanied by their husbands during childbirth would have a very positive impact on the mother's psychology because the husband's companion, the mother could communicate and shared the pain and the husband could provide calm and comfort to his wife by holding his wife's hand, teaching deep breath relaxation when feeling pain and provided motivation as well as giving attention to the wife so that the wife was stronger in undergoing during the childbirth process. CONCLUSION Husbands with primigravida anxiety levels had a significance value (ρ) which is 0.00 less than the value of = 5% (ρ = 0.05) so it meant that there were Relationship of Husband's Attendant with Primigravida’s Anxiety in the First Childbirth at Health Primary Care Centre Kunir Residence. SUGGESTION This research would expected to increase the active role of husband companion in the childbirth process so that mothers are more comfortable and calm in facing the childbirth process.
